Senior Case Manager - follow-up and outcomes (remote)

Bright Cobalt Learning Group is an education provider in Buffalo serving students and families. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for student accounts. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with student accounts. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Buffalo. • We reply to every applicant.
