Senior Case Manager - follow-up and outcomes (remote)

Bright Clear Care Services is looking for a Senior Case Manager. The job centres on follow-up and outcomes for caregiver training, and it would suit someone who likes steady, practical work. We train and promote from within. What you will do • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Savannah. • We will tell you what to expect at each step of the hiring process.
