Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Lumen Thistle Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We follow the school calendar and we like people who like learning. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Virginia Beach. • We reply to every applicant.
