Senior Case Manager - follow-up and outcomes (remote)

Northwind Maple Housing manages affordable housing communities across Portland, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease compliance. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with a monthly team day in Portland. • Applications are reviewed on a rolling basis.
