Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Summit Ivory Health Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 376 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • Fully remote, with quarterly visits in Seattle. • We will tell you what to expect at each step of the hiring process.
