Senior Case Manager - follow-up and outcomes (remote)

Lumen Ember Property Management manages apartment communities around Oakland, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 469 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with quarterly visits in Oakland. • We will tell you what to expect at each step of the hiring process.
