Senior Case Manager - follow-up and outcomes (remote)

Stonegate West Care Services Group is looking for a Senior Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Kansas City. • We will tell you what to expect at each step of the hiring process.
