Senior Case Manager - follow-up and outcomes (remote)

Lumen Sable Property Management looks after rental homes across the Reno area. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a large organisation of about 984 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for unit turnovers. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Reno. • We reply to every applicant.
