Senior Case Manager - follow-up and outcomes (remote)

Stonegate Corner Learning Group Partners is looking for a Senior Case Manager. The job centres on follow-up and outcomes for attendance records, and it would suit someone who likes steady, practical work. Families are partners. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with a monthly team day in Grand Rapids. • We reply to every applicant.
