Senior Case Manager - follow-up and outcomes (remote)

Clearpath Vista Housing is looking for a Senior Case Manager. The job centres on follow-up and outcomes for resident services, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Part-time, about 28 hours a week, mornings. • Fully remote, with quarterly visits in Cleveland. • We reply to every applicant.
