Senior Case Manager - follow-up and outcomes (remote)

Cedar Bridge Care Services provides in-home and community support in Cleveland, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 909 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Cleveland. • We reply to every applicant.
