Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Lumen Union Health Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 24 hours a week, afternoons. • Remote-friendly, with a monthly team day in Hartford. • We reply to every applicant.
