Senior Case Manager - follow-up and outcomes (Phoenix, AZ)

Westbrook Cross Civic Services is looking for a Senior Case Manager. The job centres on follow-up and outcomes for constituent requests, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Phoenix, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
