Senior Case Manager - follow-up and outcomes (Philadelphia, PA)

We are growing our team at Summit Kestrel Community Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 506 people based in Philadelphia. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for donor records. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, Monday to Friday. • On site in Philadelphia. • We reply to every applicant.
