Senior Case Manager - follow-up and outcomes (Philadelphia, PA)

Bluepeak Linden Care Services supports families and individuals in Philadelphia with personal care and coordination, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 32 people based in Philadelphia. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Philadelphia. • We will tell you what to expect at each step of the hiring process.
