Senior Case Manager - follow-up and outcomes (Philadelphia, PA)

We are growing our team at Foxglove Rook Early Learning and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 15 people based in Philadelphia. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Philadelphia, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
