Senior Case Manager - follow-up and outcomes (Philadelphia, PA)

Westbrook North Learning Group runs classrooms and tutoring across the Philadelphia area, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 29 people based in Philadelphia. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for attendance records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Philadelphia. • Applications are reviewed on a rolling basis.
