Senior Case Manager - follow-up and outcomes (Philadelphia, PA)

Ironwood Summit Community Partners runs community programmes across Philadelphia, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 116 people based in Philadelphia. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for donor records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, on a regular day shift. • On site in Philadelphia, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
