Senior Case Manager - follow-up and outcomes (Oklahoma City, OK)

Bluepeak Forge Care Services is looking for a Senior Case Manager. The job centres on follow-up and outcomes for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Oklahoma City. • Applications are reviewed on a rolling basis.
