Senior Case Manager - follow-up and outcomes (Oklahoma City, OK)

Clearpath Bright Community Partners Group is a nonprofit in Oklahoma City supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on follow-up and outcomes. Flexibility and kindness are expected. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Oklahoma City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
