Senior Case Manager - follow-up and outcomes (Oklahoma City, OK)

We are growing our team at Bright Cedar Health Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Oklahoma City. What you will do • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Oklahoma City, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
