Senior Case Manager - follow-up and outcomes (Oklahoma City, OK)

Highbridge Granite Community Partners Group is a nonprofit in Oklahoma City supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on follow-up and outcomes. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for donor records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Oklahoma City. • Applications are reviewed on a rolling basis.
