Senior Case Manager - follow-up and outcomes (Oklahoma City, OK)

We are growing our team at Clearpath Ashton Health Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Continuing education is supported. What you will do • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Oklahoma City, with some flexibility after the first few months. • We reply to every applicant.
