Senior Case Manager - follow-up and outcomes (Milwaukee, WI)

Foxglove Sable Housing is a nonprofit housing provider in Milwaukee, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance stores. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Milwaukee. • We reply to every applicant.
