Senior Case Manager - follow-up and outcomes (Milwaukee, WI)

We are growing our team at Cedar Union Care Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. The work is personal and so is the culture. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Milwaukee. • Applications are reviewed on a rolling basis.
