Senior Case Manager - follow-up and outcomes (Memphis, TN)

Northwind Cross Health Group runs clinics and patient services around Memphis, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 53 people based in Memphis. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Memphis, with some flexibility after the first few months. • We reply to every applicant.
