Senior Case Manager - follow-up and outcomes (Memphis, TN)

Bright Basin Health Group is a healthcare provider in Memphis with clinics and support teams. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Memphis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
