Senior Case Manager - follow-up and outcomes (Memphis, TN)

We are growing our team at Northwind Umber Care Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We train and promote from within. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Memphis. • Applications are reviewed on a rolling basis.
