Senior Case Manager - follow-up and outcomes (Memphis, TN)

Harborlight Pine Community Partners Group runs community programmes across Memphis, and we need a Senior Case Manager to lead our follow-up and outcomes. We are small enough to know everyone's name. What you will do • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in Memphis, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
