Senior Case Manager - follow-up and outcomes (Madison, WI)

Westbrook Maple Housing is looking for a Senior Case Manager. The job centres on follow-up and outcomes for maintenance stores, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance stores. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Madison. • Applications are reviewed on a rolling basis.
