Senior Case Manager - follow-up and outcomes (Little Rock, AR)

We are growing our team at Rivermark Ashton Care Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We train and promote from within. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Little Rock. • We reply to every applicant.
