Senior Case Manager - follow-up and outcomes (Knoxville, TN)

Foxglove Willow Community Partners Group is a nonprofit in Knoxville supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Knoxville. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Knoxville. • Applications are reviewed on a rolling basis.
