Senior Case Manager - follow-up and outcomes (Kansas City, MO)

Greenridge Cobalt Property Management is a Kansas City property manager with leasing, maintenance and resident services, and we need a Senior Case Manager to lead our follow-up and outcomes. Residents are neighbours, not numbers. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for unit turnovers. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Full-time, Monday to Friday. • On site in Kansas City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
