Senior Case Manager - follow-up and outcomes (Jackson, MS)

Foxglove Ivory Health runs clinics and patient services around Jackson, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Jackson. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Jackson. • Applications are reviewed on a rolling basis.
