Senior Case Manager - follow-up and outcomes (Jackson, MS)

Silverline Willow Community Partners is a nonprofit in Jackson supporting families with food, housing and job help. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 28 people based in Jackson. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Jackson, with some flexibility after the first few months. • We reply to every applicant.
