Senior Case Manager - follow-up and outcomes (Jackson, MS)

Clearpath Sable Community Partners is a Jackson charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Full-time, Monday to Friday. • On site in Jackson. • We reply to every applicant.
