Senior Case Manager - follow-up and outcomes (Houston, TX)

Harborlight Pine Community Partners Group is a Houston charity working with residents on basic needs and opportunity, and we need a Senior Case Manager to lead our follow-up and outcomes. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Houston. • We will tell you what to expect at each step of the hiring process.
