Harborlight Ivory Health Group is looking for a Senior Case Manager. The job centres on follow-up and outcomes for patient scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 154 people based in Houston. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish.
