Senior Case Manager - follow-up and outcomes (Honolulu, HI)

We are growing our team at Redstone Cedar Health and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Honolulu. • We will tell you what to expect at each step of the hiring process.
