Senior Case Manager - follow-up and outcomes (Grand Rapids, MI)

Nova Haven Learning Group is an education provider in Grand Rapids serving students and families, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Grand Rapids. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for school calendars. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Grand Rapids, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
