Senior Case Manager - follow-up and outcomes (Grand Rapids, MI)

Rivermark Yarrow Civic Services Group provides shared administrative services to local governments around Grand Rapids, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for budget submissions. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with budget submissions. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Grand Rapids. • We will tell you what to expect at each step of the hiring process.
