Senior Case Manager - follow-up and outcomes (Grand Rapids, MI)

Northwind Meadow Community Partners Group is looking for a Senior Case Manager. The job centres on follow-up and outcomes for grant reporting, and it would suit someone who likes steady, practical work. You would join a large organisation of about 520 people based in Grand Rapids. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Grand Rapids, with some flexibility after the first few months. • We reply to every applicant.
