Senior Case Manager - follow-up and outcomes (Grand Rapids, MI)

Stonegate Ashton Civic Services Group is looking for a Senior Case Manager. The job centres on follow-up and outcomes for constituent requests, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with constituent requests. • Bilingual English and Spanish.
