Senior Case Manager - follow-up and outcomes (Fresno, CA)

We are growing our team at Lumen Cobalt Early Learning and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 53 people based in Fresno. What you will do • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • On site in Fresno, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
