Senior Case Manager - follow-up and outcomes (Fort Worth, TX)

Stonegate Mill Care Services Partners supports families and individuals in Fort Worth with personal care and coordination. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 153 people based in Fort Worth. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Fort Worth, with some flexibility after the first few months. • We reply to every applicant.
