Senior Case Manager - follow-up and outcomes (Fort Worth, TX)

We are growing our team at Fernwood Lantern Community Partners Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 43 people based in Fort Worth. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Fort Worth. • We reply to every applicant.
