Senior Case Manager - follow-up and outcomes (Des Moines, IA)

We are growing our team at Bright Junction Housing and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Des Moines. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for resident services. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
