Foxglove Meridian Health is looking for a Senior Case Manager. The job centres on follow-up and outcomes for clinic supplies, and it would suit someone who likes steady, practical work. You would join a small organisation of about 151 people based in Dallas. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish.
