Senior Case Manager - follow-up and outcomes (Dallas, TX)

Summit Sparrow Care Services provides in-home and community support in Dallas. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 46 people based in Dallas. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Dallas. • Applications are reviewed on a rolling basis.
