Senior Case Manager - follow-up and outcomes (Columbus, OH)

Harborlight Glen Health Group is a healthcare provider in Columbus with clinics and support teams, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Columbus. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Columbus. • We reply to every applicant.
